4. CLIENT AGREEMENT

• You acknowledge your participation is voluntary and at your own risk.

Consent

I acknowledge that I am voluntarily participating in Ayurveda coaching sessions provided by Komorebi Wellness. I understand that Ayurveda is a holistic approach to wellness based on traditional Indian principles and is not a substitute for medical diagnosis, treatment, or professional healthcare advice.

Scope of Services

• Ayurveda coaching includes recommendations on diet, lifestyle, herbs, and mindfulness techniques based on individual constitution (Prakriti) and imbalances (Vikriti).
• The coaching sessions do not replace medical advice, diagnosis, or treatment from licensed healthcare professionals.
• Ayurveda recommendations are meant to support overall well-being but are not intended to treat or cure any disease.

Client Responsibilities

• I agree to provide accurate health information and disclose any medical conditions, medications, or treatments I am currently undergoing.
• I understand that it is my responsibility to consult with my healthcare provider before making any significant changes to my diet, medications, or lifestyle.
• I acknowledge that results may vary, and I am responsible for my own health choices and well-being.

Confidentiality & Privacy

• All personal and health information shared during Ayurveda coaching will be kept confidential and will not be disclosed to third parties without my consent, except where required by law.

Liability Waiver

I hereby release and discharge Komorebi Wellness and its practitioners from any and all claims, liabilities, or damages arising from my participation in Ayurveda coaching. I understand that I am engaging in these services at my own risk.

Acknowledgment & Consent

By signing this form, I confirm that I have read, understood, and agree to the terms outlined above. I voluntarily consent to participate in Ayurveda coaching sessions.
